Tremors Following Alcohol Dependency

Essential tremor usually affects your hands and arms but can also affect your head, voice and other body parts. Just as alcohol does not help many ET patients, neither do prescription medications. Even when drugs help, having to increase the dosage as tremors progress often leads to unpleasant side effects, and use is discontinued. A new treatment for essential tremor uses magnetic resonance images to deliver focused ultrasound to create a lesion in tiny areas of the brains thalamus thought to be responsible for causing the tremors. The treatment is approved only for those individuals with essential tremor who do not respond well to anticonvulsant or beta-blocking drugs. Alcohol shakes, also known as tremors, are a common sign of alcohol withdrawal.

Hyperkinetic movement disorders responsive to EtOH or GHB are listed in Table ​Table1.1. Tremor disorders appear in green, myoclonic disorders in blue, and dystonic disorders in red. People with more severe tremors struggle with activities such as cooking, using kitchen utensils, drinking from a cup, hygiene activities, grooming and dressing. Some people who have essential tremor eventually can’t live independently and need to live with family or in an assisted living or skilled nursing facility setting. The recovery time from the various treatments depends on many factors, especially the treatments themselves.

How is essential tremor treated, and is there a cure?

This noninvasive approach to treating hand tremors is an outpatient procedure that directs “beams” of ultrasound into the brain to deaden the small area that transmits dysfunctional movement signals to the hands. The result is durable tremor control, and regaining excellent quality of life. Propanolol blocks the stimulating action of neurotransmitters to calm your trembling. This beta blocker is effective in 40% to 50% of patients and is less useful in reducing head and voice tremor.

Like EtOH, GHB reaches a peak dose within 35 minutes of administration, and plasma levels show a direct, non-linear dose response. At higher doses the sedative effect peaks later (40 vs 60 minutes at a dose of 25 vs 35 mg/kg, respectively) essential tremor alcohol and decays slower, reaching baseline in no more than 3 hours [20]. GHB binds with low affinity to the metabotropic GABAB receptor, the target of the drug baclofen, as well as distinct high-affinity binding sites [21].

What are the latest updates on tremor?

We have administered the drug to more than one hundred patients in clinical trials or as part of clinical care where other therapeutic options have failed. The response to EtOH appears to predict response to Xyrem, and the pharmacokinetics and tolerability of the two agents appear to be very similar. Patients with essential tremor, vocal tremor, torticollis, myoclonus-dystonia and posthypoxic myoclonus often benefit in a surprisingly rapid and robust manner from ingestion of a modest amount of alcohol (ethanol). Despite considerable investigation, the mechanism of ethanol’s ability to produce this effect remains a mystery.

Two post-mortem studies of coeliac disease patients with cortical myoclonus have demonstrated selective loss of Purkinje cells, illustrating that isolated cerebellar pathology can generate cortical myoclonus [62,63]. In EPM-1 (Unverricht-Lundborg disease), another disorder with prominent cortical myoclonus and EtOH-response, a post-mortem study showed a similar loss of Purkinje cells with involvement of the dentate nucleus [64,65]. Many patients with EPM1 do not appear to have cerebellar atrophy on routine MRI imaging, but an MRI/MRS study of a cohort of patients demonstrated mild atrophy of cerebellar hemispheres, medulla and basis pontis [66]. Cystatin B, the protein affected by EPM1, is selectively expressed in Purkinje cells and some molecular layer neurons in the developing and adult rat [67]. In man cystatin B expression is limited to Purkinje cells and Bergmann fibers [67].

Drug treatment

Botox injections are typically recommended for patients with severe head tremor, and several studies have shown that the injections may significantly help head and voice tremors. Most patients with essential tremor experience a transient improvement after ingesting a small amount of alcohol. It has been accordingly suggested that essential tremor patients may have an increased risk of developing alcoholism. In this study, the frequency and amount of alcohol intake of essential tremor patients were found to be largely similar to the drinking habits of a control sample from the general population. This indicates that essential tremor does not generally augment alcohol consumption, nor is it a common cause of alcoholism.

While there may be few noticeable symptoms in early liver disease, prolonged liver dysfunction can cause a number of complications, including a potentially fatal brain disorder known as hepatic encephalopathy . Even though alcohol can help ET symptoms, alcohol is not usually used as a treatment for ET. Doctors do not recommend treating ET with alcohol, because there are downsides to using alcohol to improve your symptoms.

Surgical procedures may be performed when tremor does not respond to medications or severely impacts daily life. A doctor may take blood or urine samples to rule out certain contributing factors to the tremor. Diagnostic imaging may help determine if the tremor is the result of damage in the brain. An electromyogram, which measures involuntary muscle activity and muscle response to nerve stimulation, may identify muscle or nerve problems.
