In another study on this topic, Lazarević et al[23] divided a cohort of 89 asymptomatic individuals whose consumption exceeded 80 g/d (8 standard units) into 3 groups according to the duration of their alcohol abuse.

The acute ingestion of large amount of alcohol as well as chronic alcohol abuse induce toxic effects to all organs and tissues [7], particularly to central nervous system, liver and heart [8], [9]. Ballester specifically analysed the effects of alcohol withdrawal on the myocardium using antimyosin antibodies labelled with Indium-111[72]. This radiotracer has been acknowledged as an indicator of irreversible myocardial damage. Of the 56 patients included in the study, 28 were former drinkers and 28 continued consuming alcohol during the study. Absorption levels of Indium-111 were high in 75% of patients who continued drinking and in only 32% of those who had withdrawn from consuming alcohol. Unfortunately Lazarević et al[23], as in most of these studies, systematically excluded patients with a history of heart disease or with HF symptoms.

This is not surprising because mitochondria are a major target for free-radical injury; however, dysfunctional mitochondria are not only less bioenenergetically efficient, they can also generate increased amounts of ROS and are more likely to initiated apoptosis (55). As reviewed below, it is possible that mitochondria serve as a site for ethanol-induced ROS generation, but also may be a target of ethanol-induced ROS injury. In particular, mitochondrial DNA is highly susceptible to oxidative stress because of the close proximity to ROS generation and lack of protective histones and DNA repair mechanisms compared to nuclear DNA (55). Alterations caused by heavy alcohol intake have also been studied from the perspective of histopathology. Emmanuel Rubin analysed muscle biopsies from individuals who were previously non-drinkers and were submitted to a balanced diet with heavy alcohol intake during one month[41]. Although no significant changes were found using conventional microscopy, when electron microscopy was employed he discovered intracellular swelling, glycogen and lipid accumulation, and alterations in the structure of the sarcoplasmic reticulum and of the mitochondria (Figure ​(Figure2).2).

Data on the amount of alcohol consumption required to cause ACM are limited and controversial. Many changes can be observed including premature atrial or ventricular contractions, supraventricular tachycardias, atrioventricular blocks,  bundle branch blocks, QT prolongation, non-specific ST and T wave changes and abnormal Q waves.

At histological evaluation, dilatation, myofibrillar necrosis and fibrosis are typically present, with a reduction of myofibrils and giant mytochondria [3]. However, these findings are not specific and do not differentiate AC from other forms of dilated cardiomyopathy [4].
